Why Not Just Outlaw Tobacco...

Set a date certain after which the production, sale and purchase of tobacco products would be illegal. It would be more direct and honest, not to mention principled.
Allow sufficient time for the producers of nicotine replacement products to rev up, if you like--but no fair planning to tax them additionally to make up for lost revenue; that is, if you're really concerned about people's health. (Besides, allowing oneself to become dependent on the dependencies of others really is rather morally shaky, isn't? I mean, it's not all that much different than companies profiting off of addiction, when you think about it, now is it?) You can, however, phase these out later as posing hazards of their own, though fewer than smoked products.
